If you are new to Dr. Shaw or CGS Physician Services, LLC, then you are in the right place. We make getting your cannabis certification quick and easy. Our licensed physicians will guide you through the process, answer your questions along the way and provide you with the resources and support you need to make your experience as smooth as possible.
There is no transfer fee and the certification is for the maximum time the state allows, which is 7 months (210 days).
You will need to bring with you proof of residency such as a state ID, driver's license or passport.Â
If you are a seasonal resident of Florida, you must provide proof that you reside in Florida for at least 31 consecutive days in a calendar year.
The Medical Marijuana Card is renewed annually and costs $77.75. This money is paid directly to the state after you become certified. For more information or instructions on how to apply for the card, click here
